Recently, the expert team from the First Ward of Gastrointestinal Surgery at Hainan Cancer Hospital successfully removed a retroperitoneal liposarcoma with a diameter of over 20 centimeters and weighing 5 kilograms from a patient who had a recurrence after surgery. The team overcame three major challenges: a huge tumor, a rare blood type, and severe tissue adhesion.
On August 4th, Mr. Wang, the patient, was discharged from the hospital safely.

Part.1
Postoperative neglect of follow-up led to the rapid growth of the tumor.
Last October, Mr. Wang, aged 60, was diagnosed with a retroperitoneal tumor and underwent tumor resection surgery at another hospital. The postoperative pathological diagnosis was "retroperitoneal liposarcoma". This type of malignant tumor, which originates from the retroperitoneal fat tissue, is characterized by large tumor size, rapid growth, and high recurrence rate.
After the surgery, Mr. Wang was too busy with work to have regular follow-up check-ups. It wasnt until July this year that he experienced repeated abdominal distension and discomfort and went to the hospital for a check-up. To his shock, the tumor had already "come back with a vengeance" and was growing at an alarming rate.
The sarcoma almost filled the abdominal cavity and compressed the organs.
The sarcoma tightly enveloped the left kidney.
Imaging examination revealed that there was a tumor in the patients abdominal cavity, which was 20.6 cm in size and resembled a Hami melon. Multiple tumors were also found in the left abdominal and pelvic cavity, retroperitoneum, and right abdominal wall. These tumors not only adhered and wrapped around the left kidney and psoas major muscle like "vines", but also displaced and deformed the adjacent organs such as the intestines, stomach, liver, pancreas, and spleen.
If not removed in time, the tumor will intensify the pressure on the intestines, stomach and other organs, causing severe intestinal obstruction and other problems, leading to inability to eat normally, digestive disorders, and even threatening the patients life.
Mr. Wang visited several hospitals. After evaluation by experts, they all said that due to the complexity of his condition and the extremely high surgical risk, they dared not perform the operation rashly.
Part.2
The condition is complex and the surgery faces multiple challenges.
As the disease progressed, the tumor in his body grew wildly. The huge tumor began to press on his heart, causing Mr. Wang to experience breathing difficulties. He would gasp for breath after even the slightest activity, and his heart rate kept increasing. His body was enduring an indescribable torment.
After asking around, Mr. Wang made a special trip to Hainan Cancer Hospital in search of a glimmer of hope.
Professor Yu Shuyong, the president of Hainan Cancer Hospital and Hainan Chengmei Hospital, a renowned expert in gastrointestinal surgery, and Deputy Director Cai Yaoqing of the First Department of Gastrointestinal Surgery, further examined and evaluated and found that:
The tumor is not only of huge volume, invading multiple organs, but also located deep, closely adhering to major blood vessels such as the abdominal aorta and inferior vena cava.
Not only that, but also because the patient had undergone similar surgery before, the anatomical structures in the abdominal cavity had already changed. The tumor was severely adhered to the organ tissues with unclear boundaries, and the surgical operation space was extremely limited.
What is even more challenging is that the patient has blood type O RhD negative, commonly known as "panda blood", which is extremely rare in the population and has a scarce blood reserve. The blood supply during the operation is under great pressure. If the blood vessels are accidentally damaged during the operation, a large amount of blood will be lost in a short time, which will be life-threatening.
Part.3
Precise collaboration: Expert team performs surgery with "bomb defusing" precision.
The patients life comes first. To ensure the operation is flawless, Professor Yu Shuyong organized a multidisciplinary consultation to conduct a comprehensive analysis of the patients condition and formulated a detailed and meticulous surgical plan accordingly. Meanwhile, the blood transfusion department also took early action, actively contacting the Hainan Provincial Blood Center to fully prepare for the patients blood supply for the operation.
Cai Yaoqing introduced that during the operation, it was found that the situation was even worse than expected. The huge tumor almost filled the entire pelvic and abdominal cavity. There were nearly 20 tumor lesions of various sizes scattered in the omentum and abdominal and pelvic cavities, which were densely adhered to the transverse colon, stomach, left kidney, spleen and pancreas, forming a complex "tumor satellite city".
Ultimately, the expert team, like defusing a bomb, spent seven hours precisely carrying out the "tumor annihilation battle". They not only completely removed the main tumor mass but also precisely separated the tumor from the left kidney and spleen within millimeters. All visible metastatic tumor lesions were successfully eradicated in one fell swoop.
Experts warn
Cai Yaoqing stated that when retroperitoneal liposarcoma recurs, it often grows rapidly and may invade surrounding organs or metastasize, significantly increasing the difficulty of treatment. He particularly reminded that after surgery, it is essential to strictly follow the doctors advice for regular follow-up. Early detected recurrent lesions are usually small in size and have a limited range of invasion. At this time, treatment not only has a lower surgical difficulty but also can preserve organ function to the greatest extent and improve treatment benefits.
