A few days ago, an expert team of the Minimally Invasive Surgery Diagnosis and Treatment Center of Hainan Chengmei Hospital, the International Department of Hainan Cancer Hospital, implemented the "double mirror combination" of laparoscopic and choledochoscopy for a 99-year-old patient.
On September 21, patient Grandma Wang was discharged safely. The successful operation of the elderly patient reflects the comprehensive diagnosis & treatment and service ability of the hospital.
Biliary tract problems occurs frequently on the patient
Old age became a taboo in surgery
Because of the common bile duct and gallstone problems, Grandma Wang has repeatedly suffered from acute cholangitis, even causing biliary obstruction, suppurative cholangitis and other high-risk symptoms, and was hospitalized repeatedly.
Not long ago, Grandma Wang just received conservative treatment for external biliary drainage in another hospital in the province, but her symptoms soon recurred and turned worse.
Due to the fact that Grandma Wang suffers from cerebral infarction, coronary heart disease and many other common diseases of the elderly, coupled with her advanced age, functions of her heart, lungs, liver, kidneys and other organs naturally diminish, making the surgery difficult and risky. Grandma Wang traveled from one place to another to seek treatment but didn’t find an ideal solution.
However, Grandma Wangs biliary tract continues to deteriorate, if not implemented surgical treatment, it is easy to cause biliary septic shock, or induce organ failure, which is life-threatening.

The minimally invasive surgery was implemented smoothly
Grandma Wang came to the Minimally Invasive Surgery Diagnosis and Treatment Center of Hainan Chengmei Hospital to seek medical treatment. The expert team of the center immediately launched MDT (Multidisciplinary Diagnostic and Treatment) in line with the principle of "patient life first" to conduct a comprehensive assessment of the functions of the patients heart, lung, liver, kidney and other organs.
Subsequently, before the surgery, through the multidisciplinary protection, all the body function conditioning and preoperative rehabilitation training were carried out for Grandma Wang, and anesthesia experts were also invited to intervene in advance to customize the surgical anesthesia plan for the patient.
In order to ensure the safety of the operation, the expert team also formulated a number of surgical plans.
After the patient reached the surgical indication, the surgical expert team repeatedly discussed, and finally formulated the minimally invasive surgical treatment plan of "laparoscopic cholecystectomy + choledochotomy choledochoscope exploration and lithotomy".
The surgery was performed by Professor Liang Anlin, a renowned surgeon in the province. Through the perfect combination of laparoscopy and choledochoscopy, only three small incisions with a diameter of 1cm were made in the abdomen of the patient, and the severely inflamed gallbladder was successfully excised, and multiple stones in the biliary tract were successfully removed.

Due to two consecutive femoral neck fractures, Grandma Wang was in a state of constant bed rest and could not move autonomously, which brought great challenges to postoperative rehabilitation.
Professor Yu Shuyong introduced that the faster the postoperative patient gets out of bed, the better the postoperative recovery. Elderly patients like Grandma Wang that cannot move on their own, are highly susceptible to venous thrombosis and serious complications such as pneumonia and pulmonary infarction after surgery. The management of postoperative rehabilitation can jeopardize a patients life with the slightest mishap.
Fortunately, the nursing team of the hospital has created a nursing housekeeper and caregiver service model in the clinical departments with reference to the nursing experience of developed countries, so as to provide professional and fine daily rehabilitation management for special patients.
Li Yinghui, head nurse of Minimally Invasive Surgery Diagnostic and Treatment Center, introduced that facing to Grandma Wangs special condition, they worked closely with a team of surgical experts to specially formulate a post-surgical rehabilitation program to provide the patient with full rehabilitation management.
Due to cerebral infarction, Grandma Wang was severely affected by cognitive bias. Nursing caregiver Wu Kunlian introduced that, at beginning, when they patted Grandma Wangs back to remove phlegm, she would suddenly raise her hand to "fight back" and point at the caregiver who was helping her and say, "bad guy."
Later, through the "family therapy", they were gradually trusted by Grandma Wang, everyone is like grandmas granddaughter, and they were getting along very well.
Professor Yu Shuyong organized on-site ward rounds every day, and guided the operation and nursing team to adjust the rehabilitation management plan in time according to Grandma Wangs condition.
Seven days after the surgery, Grandma Wang recovered quickly and was discharged safely.
